New Forms for Determinations

The DOB has updated the CCD1 and ZRD1 forms, which are used to document official Determination Requests for DOB review.  Zoning Resolution determinations may be requested with the ZRD1, and all others with the CCD1. Generally, these forms should be used for objections raised by a plan examiner, or a possible future objection for an application which has yet to be filed.

The new CCD1 and ZRD1 forms went into effect on February 16, 2016, and requests to review the  Determinations will only be accepted by email, to the following addresses:
